I agree, Zoho Survey would fit the bill, and be a great alternative. I have tried it, and it is very professional and easy to use. You can export the results in formats such as CSV and SPSS.
I should have gotten a Zoho email account when it was free a few years ago, after a friend switched to it. At least at that time they claimed they don't scan your emails and don't use your info for marketing. That's probably still true because it's a paid service. It's not that expensive for the first level, I guess, except I don't need even their minimal suite of services, aside from email.
Still asking around, but Cryptpad and Mailfence Documents look promising from a privacy standpoint. Cannot vouch for functionality.
Having the Google account requirement to complete is what is limiting to the more paranoid types; it's the massive funnel into the Security State Ministry of Truth Social Credit Score.
Do I think that other services are bulletproof? Doubtful, but Google just funnels everything straight to the same people who don't really want you to know about people dying from Forced Injections.

Are you aware of the Vaccine Control Group in the UK? They are collecting data on about 300,000 people around the world ( of which I am one) with monthly reporting of health status, vaxxed or not vaxxed. Their data set might be of value to you.

One had been in remission for 15 years and the other a year and was told she had less than a 1% chance of her cancer returning. Not only was their cancer back, but it was very aggressive. The one friend who had been in remission for less than a year died 6 months later. The other friend who had been in remission for 15 years went through chemo and was back in remission by the end of summer 2021. Her oncologist told her to get the booster, and within 5 weeks her cancer was back and had spread. She died 4 months later.

I don't remember the exact day of the month, but I do know the month in 2021. Two people unvaccinated but died on vents after being told that they were improving and were being weaned off the vent. Both had different, well controlled autoimmune diseases. One was advised by dr to not take the vax and the other didn't want to.
Steve, this survey seems to have a major flaw. It NEVER asks if the person who died ever had covid itself, it only asks that in the context of death, but to be fair to the vaccines, we should compare ACM of everyone who had and recovered from covid to those who were vaxxed. People will use that lack of question to poke holes in the results. You should ask if the deceased had ever had covid to fairly demonstrate the negative efficacy.
Going to a friend's funeral this morning. He died at 55. He developed a mysterious aggressive lung disease diagnosed in October when docs gave him 2-5 years to live. We were all shocked then, but more shocked that he passed so soon. I'd never heard of "interstitial lung disease" and then I looked back and realized that an old acquaintance who'd died of a rapid onset debilitating lung destruction in July '21 at 44 years was also diagnosed with "interstitial lung disease". BTW both guys who died of lung disease had two things in common: a previously diagnosed "auto immune disorder" that they may have been treated for and because of that higher risk condition, they each sought out a protective injection they were told would protect them. In the movie version where the good guys win, the doctors are typing away and sharing data with their peer group scratching their heads over these individuals' deaths, and somewhere in the Bethesda, MD area, a well compensated researcher at NIH or CDC is poring over data and dramatically realizes there's an issue hits send on an email escalating the issue. Not to be ignored, s/he then runs down the hall and bursts into an executive meeting and says "I've found a pattern!" They immediately get the higher-ups from Operation Warp Speed in the FDA and private pharma leaders on a call and they all say "Ohmygosh, when Bob died last year we thought it was an oddity but now that Bill and umpteen others died or are dying, we see a pattern. We need to preserve lives and get this info out there immediately to every doctor in the nation!" I'm just a professional guy but I can see the patterns. I know close family friend who died from Covid (72 y/o who'd had leukemia/chemo). In the last year I know a friend's brother -45 y/o who died on his lawn tractor (had massive arterial clotting heart attack)... Our controls engineer in my last job died from a heart attack w/in a month last year...I know a 56 y/o who had to be out of work for 3.5 months with Guillain Barre (got paralyzed on vacation to Hawaii- what do you have to get tuned up to go there?)... My mom developed PMR full body joint pain and swelling suddenly a few weeks after- her doc told her good news, she was eligible for first in line for booster bc she had an autoimmune disease, she and her doc were perplexed that she had a major flareup after boosting... my older sister developed rheumatic arthritis after... my wife's friend who is a surgeon was jogging and passed out and was disabled... my wife's coworker in her mid-30's went into menopause immediately after (and worse, her doctors and others treat her like she's crazy for suggesting a correlation), friend's wife passed out and fell... Dad got debilitating vertigo for 3 weeks after second... I could keep going, because there are so many odd coincidental disabling ailments and working-age deaths I can't keep track of them. He was vaccinated against his will, and developed blood clots in his lungs. His health worsened after he was put in a facility to recover, put on oxygen, and 11 drugs. He came home but was not doing well at all. He went to different hospital that didn't like how he had been treated. They did a much better job, but it was too late. He was taken off everything and put in hospice to die. He didn't recognize his own son, was unable to walk or go to the bathroom, was wasting and not eating. After being off all the crap they had him on for over a month, he suddenly popped back to himself and started eating and is even walking a little. He will be out of hospice soon and is going home. Whatever they had him on was killing him. They shortened his life, he still has blood clots in his lungs, but he still has living to do. Our medical system is back in the dark ages. He would have been better off with no treatment at all. He said he would rather die at home than go to a hospital again.
