If someone you personally knew died after December 1, 2020 from ANY cause, please take it now. It will only take a few minutes. Read the instructions on the survey form before filling it out.
The more details you know the better. If the person was vaccinated with a COVID vaccine, it is extremely important to know the date of the last COVID vaccination.
This is hugely important. I’ll explain why later.

Not for your survey but a positive story: my 90-year old mother and I, both unvaxxed, tested positive earlier this week. I had managed to get some horse paste last year and we've just finished our 5-day course. Neither of us have any pre-existing conditions and have been on Vitamins B, D and Zinc for about a year supplemented with homemade hydroxycloriquine. We are both fine, still a bit tired as this coincided with the onset of our first heat wave. Grateful that so many brave doctors and others provided information and advice for early treatment.
