Don't write "STOP HERE" in first miscarriage question. Instead, write "skip to the last question" (Anything else you want us to know?). Are you limiting this survey to death and miscarriage vice other adverse events on purpose?
I think the question asking if the respondent believes the vaccine is both safe and effective may be too black and white, and risks being hard to answer. One could get stuck wondering "What exactly do you mean by safe? nothing is perfectly safe." or "how effective and for whom?". An alternative way to ask could be the following: in general, do you count yourself among those who find the vaccine mostly safe and effective, or those who have doubts/concerns about the vaccine's safety and efficacy. There are, of course, many other ways to possibly ask it, like using a scale of 1-7--how confident do you feel in the safety and efficacy of the vaccine on this scale. This might make analysis more complicated but could help address any issue with the yes or no question. Thanks for doing this. I think it's great overall! it would not be a major issue if it stays as is.
I know two elderly people in a Christian group who died before the vaccines rolled out; one had multiple co-morbidities who died of covid on death certificate the other had lung cancer who was diagnosed too late.

Just learned a coworker passed away on May 18th. From "Extremely sad to know about his sudden demise" from his manager, it's a sudden death. I don't know whether it's vaccine related, but I highly suspect.

You can file a complaint with the FCC. Their website gives all the details. It's probably spitting into the wind, but you never know. And to answer your question, they're complicit, not illiterate. Just follow the money trail to see who is actually paying for the COVID injection propaganda that is masquerading as advertising.

Later he was found dead. His son set up a Gofundme account for his funeral. You did miss the point. The surveys require something to do with Google & I have my browser set up to BLOCK anything Google- Captcha, docs, maps...
