Really thoughtful survey. May I suggest that you could add the following question. "How many people do you personally know who have been vaccinated and then diagnosed for cancer? Options could range from (0,1,2,3, more than 3)

I know of a somewhat unique story that falls under this one. It seems unique because the person had just gotten a yearly checkup and the doctor said their bloodwork was perfect in every area, but while they were in the office convinced them to get the vaccine. Within two weeks they developed a persistent cough. It was chalked it up to allergies since it was spring. After two months the cough was still still there and getting worse so they did some tests and found a tumor in their lungs. New bloodwork was all screwed up and they were diagnosed with metastatic melanoma stage 4. They had cancer in areas besides just the lung. It is hard to imagine that could be caused by anything except the vaccine.
